I am participating in classes or services during which I will receive information and instruction about any of the following; yoga asana, ayurveda, reiki, energy healing, guided meditation, breathwork, therapeutic movement, wellness. I understand that;

1. a session/class may include any of the above mentioned; both active and restorative/therapeutic, with or without hands-on adjustments, potentially strenuous physical movement, an opportunity for gaining flexibility, strength and the development of cardiovascular systems. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated and I am fully aware of the risks and hazards involved.
2. If I experience any pain or discomfort, I will listen to my body, adjust the task and ask for support from the teacher.
3. I affirm that I alone am responsible to decide whether to participate in a session/class and I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of my participation.
4. Any information shared with me is educational and I understand it may not be the best fit for my personal situation and shall not be construed as medical advice, not a replacement for professional medical advice, diagnosis and/or treatment.

I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against ML Wellness LLC/Mimi Le.

1. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness program, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities, and I will share relevant information about my health with my Instructor concerning any past/present medical condition/history that could affect my practice.

I pledge to refrain from using personal devices if possible. Any inappropriate behavior will result in immediate termination of the session and the client will be charged for the full price of the booked service.

Mimi Le has the right to cancel a session and reschedule due to illness, personal emergency or if there is inclement weather.
